Population Health Management
Population health is dedicated to creating a fundamentally different culture
and perspective focused on wellness, a paradigm shift that leads to changing
health choices both at a conscious and unconscious level -- for care providers,
payers and consumers. Refining and improving the way we deliver care through
collabora- tive partnerships and prevention programs that address the
health care needs of our community are critical to improving outcomes
in a cost-effective manner.

Dr. Sushma Reddy will continue as Chair of the Diabetes Excellence Team
and develop expanded services to address prevalent chronic illness throughout
our community; including an intensive diabetes management program, a continuous
glucose monitoring clinic, a thyroid cancer and radioactive iodine treatment
clinic, osteoporosis prevention and treatment as well as a fracture liaison
service. These programs will complement the primary care provided by a
patient's physician and work hand-in-hand with our clinically integrated
network to achieve optimal quality outcomes.
Dr. Reddy's appointment as our Director of Population Health is another
meaningful program that will ultimately improve the health of our community.
Dr. Sushma Reddy takes great pride in being available to her patients and
colleagues in the community. She is committed to building strong relationships
with every patient based on mutual respect and open communica- tion. She
works to inform and educate her patients in order to provide the best
possible treatment plan.
Dr. Reddy provides services such as diabetes management and education,
insulin pump therapy, thyroid, adrenal and pituitary disorders, osteoporosis-bone
and calcium disorders, and polycystic ovarian syndrome (PCOS).
On staff at Lake Huron Medical Center for more than 25 years, Dr. Reddy's
professional roles include a wide range of experience. Previously she
served as Vice Chief of Endocrinology, Department of Internal Medicine
Grace Division at Harper-Grace Hospitals in Detroit, Assistant Professor
of Medicine, Division of Endocrinology and Metabolism at Wayne State University
in Detroit, as well as her private practice in endocrinology care.
Dr. Reddy earned her medical degree from Kakatiya Medical College and completed
a rotating internship at Mahatma Gandhi Memorial Hospital. She completed
her residency program in Internal Medicine at Wayne State University and
completed her Fellowship training in Endocrinology and Metabolism at Wayne
Current Population Health Management programs offered include:
Intensive Diabetes Management Clinic
A multidisciplinary patient centered team (RN, CDE, NP and MD) approach
to improve glucose control over a 12-16 week period with a goal of A1C
< 8% or a 2% reduction and patient empowerment. Patient needs to be
willing to commit to weekly to semi-weekly appointments.
Diabetes Education/Medical Nutrition Therapy Consults
Individual and group Classes, for those patients newly diagnosed with diabetes
or in need of information to manage their diabetes with emphasis on self-management
goals. Medical nutrition therapy consists of instruction from our registered
dietician for issues involving nutrition.
Continuous Glucose Monitoring Clinic
Patient wears minimally invasive monitor placed on abdomen for 3-7 days
with results and recommendation by Dr. Sushma Reddy.
